how to refer a patient
crisis services
If you have a patient in crisis, please send them directly to our crisis services department for immediate assessment.
We are open 24 hours a day, 7 days a week at 8535 Tom Slick, San Antonio, TX 78229. Our licensed staff can determine the level of care that is needed.
You may contact us at 210-582-6412 to provide any information you feel may be helpful in the assessment process.
outpatient services
Clarity Child Guidance Center’s Outpatient Services include:

child, adolescent, and family therapy

developmental assessment

psychological assessment
-
psychiatric evaluation and management

neuropsychological assessment
If you have a patient who is not in crisis and could benefit from our services, please have a guardian call 210-616-0300 and ask for Outpatient Admissions. This extension is answered Monday through Friday, 8:30 am to 5:00 pm.
The guardian will need the following information to schedule an appointment:

who referred them to us
-
current living situation (custody situation)
-
treatment history of the child (past and current medication/hospitalization)
-
insurance information
