Is your child CURRENTLY at Clarity in the Partial Hospitalization Program?
Please notify the partial nurse at drop-off or pickup or reach out to a care coordinator for further assistance.

If your child has discharged from Clarity, have they already been enrolled with an outpatient provider for medication management?
- YES, but child will run out of medication before their appointment. Please call the care coordination department at 210-447-6937.
- NOT SURE or need further assistance with resources. Please call the care coordination department at 210-447-6937.
- No. Please call primary care provider’s office for assistance.
*Please note: An outpatient medication management provider is licensed to prescribe medication and continues to manage care for patients after they have been discharged from the hospital. Healthcare professionals such as Doctor of Medicine (MD), Doctor of Osteopathic Medicine (DO), physician assistant (PA), or nurse practitioner (NP) are among the providers that can provide medication management services.

Please call SPP and leave a detailed message including the patient’s name, date of birth, prescription need, and a call back number. Please allow 2-4 business days for your prescription to be sent directly to your pharmacy.
UT Health Outpatient Clinic

Please call the UT Health Outpatient refill and leave a detailed message including the patient’s name, date of birth, prescription need, and good call back number. Requests are processed on Mondays, Wednesdays and Fridays at 3pm. Messages need to be received by noon to be processed on the same day.
