Following a patient's discharge, clinical documents for the aftercare appointment are faxed to the provider that is completing the aftercare appointment. This step does not require a separate request or parent/guardian signatures.
Medical records can be requested and received within a 15 day window by faxing the request to
210-507-7675. The request MUST include the signature of the parent or guardian authorizing the release.
We’ve found that many requests do not include the parent or guardian signature, and records will not be released without that critical information.
The only time a delay of up to 30 days will occur is when the patient was discharged and the records are not complete; we do not send partial records and will await record closure prior to sending.
